Assessment & Care Management Worker
About the Position
An opportunity has become available to join the highly respected assessment team within Council’s Home and Community Care service.
The purpose of this position is to undertake comprehensive Living at Home Assessments for frail aged p eople, people who live with moderate to severe disabilities and their carers to ensure access to appropriate and quality-driven care and service provision within the Active Service Model.
You will have significant experience in the provision of assessment and care services, and will possess excellent communication, interpersonal and problem-solving skills. You will have a good understanding of primary care services, in particular the aged and disability sectors as well as resources within the Ballarat community. The successful applicant will be a strong team player, work within a strength based framework coupled with a genuine desire to ‘make a difference’ through their work with members of our community.
This is a full-time maternity leave position until April 2013, with a salary between $64,038 and $69,178 per annum, plus 9% superannuation.
Please note: Applicants will also have to demonstrate they have a current Victoria Driver’s Licence or demonstrated mobility access.
